SMART Notes for Patient Management
The SMART note is a document given to the patient to define roles and responsibilities of the clinical relationship. The patient's desire for specific answers and expectations is balanced by the clinician's ability to establish a structured care plan and hold the patient accountable for non-compliance. There is particular emphasis on selection of measures most appropriate and practical to the clinical setting.
The SMART note is the natural compliment to the SOAP note.
Curriculum
Before Treatment Begins
Establish the rules, expectations, roles and responsibilities during the contemplation stage. The time when clinician and prospective patient first meet.
During Care
Learn to select valid and reliable measures which are practical in the clinical setting. Ensure the patient understands and is committed to their "homework". Set criteria and deadlines for success.
Practice Management
When standardized treatment is accumulates the clinician may compare patients, develop their own protocols and consider sharing their results with others... maybe as case studies.
